Single Pet or Owner Transfer $19.95
Multiple Pet (3 pets) $50.00
(Save money with the multiple pet registration. You can register a single pet at this time and add 2 additional ones in the future)
Please do not send multiple copies of your registration form. Payment can be made by credit card, check or money order. Applications sent by fax must include credit card information in order to be processed.
Once your application is received, the registration process takes up to 2 business days to complete. You will then receive a data verification letter by mail. Please be sure to check for accuracy. You should receive your verification letter within 1-2 weeks of registration.
